I’ve known Population Health Management Advisors’ CEO, Suneel Ratan, since we worked together from 2013 to 2015 at Community Health Center Network in Alameda County. There, he led the team that developed one of the first FQHC-based complex care management programs, which continues to this day. I respect the expertise, experience, and passion that his team brings to the cause of turning population health management from buzzwords into real programs that are sustainable and impactful. I know they’ve supported their initial clients in doing great things and are always up for taking up the biggest challenges and that California’s CalAIM initiative reaches and supports the most vulnerable individuals and populations.

My team at Wellness Equity Alliance takes on the hard stuff ranging from the homeless to tribal health – marginalized populations that get lost in the cracks of our public health, health care, social services and criminal-justice non-system. Providing health care to the homeless on the street is still maturing and the idea of street care management – longitudinal interventions to meet the homeless where they are and get them to a safer and healthier place – remains largely conceptual. We’re lucky to work with Population Health Management advisors to develop a scalable and effective model of street care management so we can impact the lives of thousands of homeless Californians.
